Staff and accommodations have been superb.

Seems like my friend is being well taken care of. No complaints from her or me. I really do not want to leave you a review every time I visit my friend.

As a current patient of Sage Creek rehab my RN Angela has been the finest care giver I have ever encountered and she always goes the extra mile. God Bless you Angela!

So far my husband has been in the facility its a hit and miss so far regarding the attention he needs and for him doing PT and OT they stuff so many people in at one time where patients do not get the opportunity to have one on one with therapist which is sad He now feels so discourage where you should never feel that way being in a facility you all need to seriously change your procedures and please hire people who have empathy towards the patients not hire people just for a paycheck last time I checked Nurses and anyone in the medical field should be doing this job to help people not make people feel like they are a burden.

This is our first experience with a skilled nursing facility and everyone made us feel comfortable and confident with a clear plan for my mother.

Everyone was very friendly, in good holiday cheer! I spoke to Paige to introduce her to Arosa and what care management services, we provide. I stopped in without an appointment. Paige was amazing and took time to speak to me during her busy day.

Very friendly and helping staff

I just want to thank Tyler and Ezra for they have been incredible nurses to my Aunt Adelina, caring for her with genuine compassion and professionalism every step of the way. They consistently follows through on all the responsibilities that help her recover safely—monitoring her progress, coordinating her therapies, managing medications, educating our family, and making sure she feels supported and encouraged each day. We are truly grateful for their dedication and kindness. Tyler and Ezra made a difficult time so much easier, and our family deeply appreciates everything they’ve done.

All the people are very helpful

In general, Sage Creek has been serving my wife quite well. The staff is pleasant, her room is excellent, medications are dispensed promptly, and the food is very good. My wife has needed to have fer position on the bed changed regularly. This is something that was not provided as was necessary. I highly recommend Sage Creek.

My mother Ms. Brewster, received the best care from this facility. She came from the hospital after leaving Coronado ridge nursing home where they really messed her up badly. Sage creek took such good care of her and continued to call and care for her well after she was discharged to a long term facility making sure she is taken care of. I would like to especially thank Richie for being an excellent coordinator! He really cares and it show’s. I don’t know what situation I would be in without you. Thank you Richie

I had the best experience with the transition of my mother from sage creek with a man named Richie! he went out of his way to find a home for her and made sure all was well. I will be willing to let others know about his great service.

my mom is in here currently..ive been trying to call the number on the website and no one has answered the phone. its been a week straight and nothing..everytime i call it says everyone is bisy smh.

Sage creek is not a good place. My mother has been in the facility for a month. The staff does not come in a timely manner when she puts the call light on. They dont give medication on time they are allowed to do whatever they want whenever they want. I went to the director of nursing to nip the situation in the bud and now the nurses take it out on my mom because I complained about how she was treated. I don’t understand how these facilities are allowed to get away with whatever they want to do. They are constantly out of supplies make the patients bring there own when medicare is paying for them to be in there. I dont expect them to drop everything but I do expect them to come within a reasonable about of time not 45 minutes or even up to am hour later. The gossip to other nurses when you do say something the Healthcare system is failing us all

The food is horrendous. My partner is starving, unable to eat the tasteless mess. The staff are very very nice, doing their best, but communication regarding care is inconsistent and unreliable.
