If you care about your loved ones , don’t even think about bringing them into this place. It’s a hello. Substand equipment.\nInadequate staffing , outdated furnishings , both decorative and functional. If you need something from a staff member expect to wait hours or longer. And it shouldn’t have a three star rating , it should have a one star rating

While in town I visited a dear friend who was a patient at Sage Creek Post-Acute. I saw my friend two days and both days I was very impressed with how immaculate the facility was. It was nice to see how friendly the staff was from the moment I entered the building. The electronic sign in/out process was simple. On the second day that I visited, I brought my friend some flowers and desperately needed help with them because I didn’t have a vase and Kate (my friends nurse) was extremely hrlpful and found something that worked. I really appreciated her kindness. The building looks fairly new and the staff seems to be caring and professional. My friend has been there 6 days and she is pleased with her experience there.

Great staff, I wished they had PT and OT on the weekends.

It’s a nice looking place but my Mom was soaked through all the bed sheets When I arrived I could smell it I cleaned her up but the shower spray nozzle leaked and spewed everywhere there were no towels anywhere that took a while to get some towels while she froze waiting . The bell response was pretty slow she wouldn’t have made it in time anyway . Unfortunately the next 2 days when I arrived she was again soaked to the bone . Luckily we got her food changed to something that she could eat It was pretty sad seeing her that way and even sadder that she wasn’t getting the attention she needed I wasn’t impressed they seemed short handed and unaware of her need for assistance to the restroom. She was incredibly week and seemed over drugged to be quite honest The Staff were friendly I can say that just a bit clueless on what was going on .I am not posting the pictures to spare you!

PROS: Enjoyable experience considering how compromised my wife was with cancer complications. Food was great, really great. PT & OT staff was really good and the gym was well equipped. Nursing staff was fantastic and we felt confident in their care, response time, scheduling and execution of tasks and monitoring. Environment was lovely…not overtly institutional and decor was well done. Very pleasant. Allowing a loved one to stay overnight and pup visits were really appreciated. Great location. Cons: Little to complain about, really! Attention to asthetic detail is needed to spruce the place up…touch up paint in the rooms and new carpet in the common areas would go a long way. Busy gym/workout room is not laid out as well as it could be – too much is taken for admin space but it gets the job done so again, not a deal breaker. Office hours end at 5pm and that was inconvenient a few times…being open until 6pm would have helped but we adjusted. None of these observations take away from the overall experience, for sure. It is an excellent choice. We have not settled our tab yet with insurance so I cannot offer comments on price/billing/accounts payable areas. We desperately needed this level of care and assistance for the month we were there for her, so it matters not to our family. Best wishes for all who need this level of care and Sage Creek is a terrific choice overall.

We went in to see my girlfriends brother. He is being well taken care of by Ann. Ann has a great attitude. Also, the facility is clean and well taken care of. The place if quite to let the patients rest.

Fair at best. The staff , most of them were great, when you could find someone as it appears they are severely understaffed. The food was ok. Again, some really great people , but some who just don’t care. The carpet in the facility is dirty and needs to be replaced. Patients are allowed to sit in the nurse’s station and one patient was allowed to sleep on the couch in the common area.

My brother n law is getting the best care and support

Yikes!! Would never want anyone I know to go here. Especially be driven around by their public transportation. Aggressive drivers, beware!!

Loving the care my friend has been receiving while staying here! Nursing has been super caring and accommodating, transportation has been made for appointments, therapy dept has been super helpful and encouraging with exercises and suggestions for strengthening, activities dept have been offering daily events to keep everyone engaged and busy, dietary staff has made some really lovely looking meals! Thank you EVERYONE for all you co tongue to do!

Beware of this place. My loved one would have died had I not forced them to send him back to the hospital. Beware if a person named Eduardo Ramos that calls himself a dr. Neglected my loved one and took measures that put him back in renal failure. Complete medical neglect. Watch your loved ones closely if they are here.

I had a really positive experience with this facility. The staff truly made me feel cared for and supported, and they were always kind, patient, and willing to help. You can tell they genuinely care about the people they work with. I’m very grateful for my experience here and would definitely recommend them to anyone looking for a supportive and welcoming place.

Hey, just wanted to say my grandma has been here twice and throughout all the visits, this place has been kept throughly cleaned and still resembles as fairly new. The nurses that I’ve ran into so far are all nice, and caring to my grandma, nothing bad to say so far. Well done Sage Creek 👏 THANK YOU!!

Great rehab facility with a caring and supportive staff from the receptionist Doreen to the case managing department, including Paige, Danielle, and the assistant director of nursing Marlena. Have to give kudos to the nursing staff as well. The facility is clean, welcoming, and provides excellent care. I would highly recommend them to anyone looking for a smaller, more personal rehab facility.

Great facility to go for therapy and skilled needs

This facility is wonderful. The staffs there are so compassionate, patience…and caring. The staffs are are always on top and communicate effectively when needed .
