Loving the care my friend has been receiving while staying here! Nursing has been super caring and accommodating, transportation has been made for appointments, therapy dept has been super helpful and encouraging with exercises and suggestions for strengthening, activities dept have been offering daily events to keep everyone engaged and busy, dietary staff has made some really lovely looking meals! Thank you EVERYONE for all you co tongue to do!

The Director of Nursing is awesome he went above and beyond

I had a really positive experience with this facility. The staff truly made me feel cared for and supported, and they were always kind, patient, and willing to help. You can tell they genuinely care about the people they work with. I’m very grateful for my experience here and would definitely recommend them to anyone looking for a supportive and welcoming place.

Hey, just wanted to say my grandma has been here twice and throughout all the visits, this place has been kept throughly cleaned and still resembles as fairly new. The nurses that I’ve ran into so far are all nice, and caring to my grandma, nothing bad to say so far. Well done Sage Creek 👏 THANK YOU!!

Great rehab facility with a caring and supportive staff from the receptionist Doreen to the case managing department, including Paige, Danielle, and the assistant director of nursing Marlena. Have to give kudos to the nursing staff as well. The facility is clean, welcoming, and provides excellent care. I would highly recommend them to anyone looking for a smaller, more personal rehab facility.

Well this is tom Shaw , you blocked me from any further comments so I’ll continue to use other phone lies. On Monday at 9 am I walked in and found your director of HR sitting there. I gave her. An earful. She escorted me oaesy from her office towards my wife’s room. She said she had a meeting. I told her I wanted to talk to the attending, director of nursing snd a case manager. No one showed, they sent another case manager from empower at 1:00. She had my wife removed to a top tier facility by 6. Fire your director of nursing and HR they didn’t have the respect for your facility to meet with me. And you, sent mea general number. Ha. I have an appointment with BBB tommo

No sorry I’m not going to call you, I walked in at nine Monday morning and yelled at the director of HR. I said I want to see the attending, director of nursing and a case manager. No one showed up all day they sent me a case manager from empower which is a is a great place. Moved her by 6 tonight. Your place is a ghetto, she frequently my wife frequently layed In dirty diapers, the pillow case had blood on it, the same blood for four days. Your human resource manager and director of nursing should both be fired

My wife has been left in dirty diapers multiple times and I have driven here all the way from Inspirada and still no answer to the bell. 10:30 am and 2:30 pm. Just today. I came in and give nurses sitting around in the nurses station and her room 55 is three feet from it and it was until I yelled that she got help. One nurse said we don’t know who her CNA is. Excuse me, all five of you are supposed to be providing patient care! I’m moving her out tomorrow morning, even a third rate place is better than this. The time shows her initial call to me was 2:40. The video shows the nurse with diapers at 3:10. Add the 15 minutes she said she pushed the button before calling me.

Very friendly people! I was waiting for my loved one to get done with his dialysis. I was in his room and the CNA Addison offered food when I mentioned I was hungry. She offered a sandwich but brought back a sandwich, chips, cookies and ice water! So kind. His nurse was nice. The gentlemen who brought him back to the room was great. The speech therapist came by, she was also nice. Wound care lady… nice. 😞 My loved one had no complaints! 💜

Doreen always had a smile and is very helpful. I had a great experience with Danielle for transportation, and the charge nurse Kat was able to answer all my questions and I had a lot of them. Mark and Marlena were always walking around with a smile and a kind word keeping things in order. It was a great experience helping my uncle get through rehab with his broken hip.
