Our grandfather took a tumble and spent some time with the awesome staff at sage creek. How happy he was with the care and service eased our minds of the displacement. Hed live there if he could! Thanks for taking such good care of our poppy pop.

Sage creek has been an absolute blessing to my family. They’ve taken the best care of our sweet titi. The nurses, the doctors, the therapy staff and especially the administrative and support staff. They’ve made us feel so welcome and go above and beyond to make all us feel comfortable and confident in the care they are providing.

I cannot recommend Sagecreek. The facility lacks organization, cleanliness and tact in handling matters. There are some good people, but understand they are all contractors which is why if you ever wondered the place seems aloof in handling your loved ones in a timely manner or you find yourself doing their jobs when there and worry about your loved ones when you leave. The state accepts bare minimum care and they meet the minimum standard barely, so do your research and know how all facilities work in the state of Nevada and what the state allows, which appears to be everything short of death. Profit over patient care, its everywhere, but you have choices, dont accept minimum standards.

This place from the very beginning welcomed us in and gave us peace. After my dad had a stroke, he was at a rehab facility. Medicare determined it was time for discharge. They had encouraged us to discharge home, but my dad was still unable to walk, get out of bed, roll over or another kind of independent action at that time. We talked to Tammy at Sage Creek. She was so calming and reassured us that my dad would have the care he needed, they could provide transport for the office visits he needed for his heart surgery and would have the PT, OT and SLP that he needed. We were told at the rehab that a skilled nursing facility was going to just leave him in bed and he would lose any progress he had made with them. This is absolutely untrue for this facility. They got my dad up and dressed every day. He had therapy. The therapists were so patient and kind (Alexa, Vicky, Willy and all the others I’m so bad with names) Mo the nursing supervisor went above and beyond to take care of any concerns that we had. Judy, Tyler, TJ, Brandi, Lindsay, CC, Rosann, Malik and other nursing staff were all amazing. The facility doctors were wonderful with communication and the care they gave. The facility was clean and the food was good with lots of options. Danielle and Nicole in transportation were amazing taking my dad to his many doctors appointments. Paige, our case manager, worked so hard for us. Anything we need to get my dad in the VA Home she came through for me. Absolute godsend! We adopted the staff here as family and we already miss them. Their kindness and thoughtful caring was beyond any of our expectations. I 100% recommend this facility

We are very happy with the services and the care that our mom is getting from the nurses and the employees of Sage Creek facility. They are very sweet nice knowledgeable and the facility is clean I will recommend it to all of my friends and family thanks for being awesome

I give you my full review if my sister comes out of there alive

Most of the staff we encounter are great/friendly! My family member has been there for a few weeks now and has memory loss. When she presses the call light it takes forever for someone to come. I get there are other patients they are assisting. By the time we get to her room she’s already made a mess. One nurse Angela told my family member to have her calm down 🤷🏽‍♀️😠. This is unacceptable my family member doesn’t know what’s going on she’s in an unfamiliar place and gets scared. I want to thank the ones who have been there for her, Jeremy, Shere, Esperanza, Michelle.

A well kept facility with a friendly and attentive staff

I don’t know who the Doctor is for this facility but what he has done with my 76 years old prescriptions almost killed her Nobody there listens to me and I don’t know how to hold of this mysterious doctor who I have never seen 😞 Linda Ferguson Regarding Wanda Starnes

I have visited Sage Creek 2 times. My Neice is a patient there. She seems to be comfortable and the staff is nice. She is able to walk around and can speak for herself. She’s not afraid to say what she Thinks if she is dissatisfied! So far so good!

My grandma was getting care at this facility for a bit and we couldn’t be happier! Great staff and a professional environment.

Family member had the best nurses and care. Loved her stay here and had the best rehab therapists.

Terrible food and smelled. Was there for 3 months. Still couldn’t walk after all that time

Horrible care. Nurses don’t know what they are doing and constantly blame one another (morning vs. night shift). Told them of what medication to avoid for my mother’s care which was not inputted properly and caused her pain. Escalated issue to nursing director with no response.

There is a smell when you immediately walk in the door. It smells like poop but from my understanding it was the sewer line. This is like the 3rd time I’ve smelled it.

Staff and accommodations have been superb.

Seems like my friend is being well taken care of. No complaints from her or me. I really do not want to leave you a review every time I visit my friend.

As a current patient of Sage Creek rehab my RN Angela has been the finest care giver I have ever encountered and she always goes the extra mile. God Bless you Angela!

Kiesha was wonderful help my family member with everything that they needed all I hear is how great she is as a person and a worker keep up the great work sage creek need more staff like her
